Give The Gift Of Health

1. This gift voucher is only redeemable at Epoch Lincoln for the services specified on the gift voucher, by the expiration date indicated.
2. Gift vouchers have no monetary value and cannot be exchanged for cash.
3. Once purchased, gift vouchers cannot be refunded or returned.
4. If a gift voucher is lost, stolen or destroyed a replacement will not be provided in these circumstances.
5.Please present this voucher when attending the Initial Consultation and Exam visit. A minimum of 48 hours notice is required should the Initial Consultation and Exam appointment need cancelling or re-scheduling. If less than 48 hours notice is provided the voucher will become invalid and full payment will be required should the appointment be re-scheduled.
